Abstract
Transanal minimally invasive surgery (TAMIS) and transanal total mesorectal excision (taTME) are important development directions in the minimally invasive evolution of rectal surgery. Transanal endoscopic procedures are often constrained by confined working spaces and the "chopstick effect", resulting in high technical challenges and steep learning curves. Robotic transanal minimally invasive surgery (R‑TAMIS) features high‑definition 3D vision, flexible robotic instruments, and superior maneuverability within narrow spaces, which provides a novel solution to the technical challenges of laparoscopic TAMIS. To further standardize the definitions, indications, contraindica-tions, and operational workflows of R‑TAMIS, and to promote its safe and effective implementation, the Transanal Endoscopic Surgery Expert Working Group of the Chinese Medical Doctor Association, the Robotic and Laparoscopic Surgery Committee of Chinese Research Hospital Association, and the Editorial Board of Chinese Journal of Digestive Surgery have organized experts in the field to develop the Expert consensus on robotic transanal minimally invasive surgery (2026 edition). Integrating both domestic and international evidence‑based medical data, this consensus aims to provide scientific guidance for the standardized practice and clinical promotion of this technology.
