Nemo样激酶表达与胆囊癌侵袭转移及患者预后的关系

Relationship between the expression of nemolike kinase and the invasion and metastasis and prognosis of gallbladder cancer

  • 摘要: 目的:探讨胆囊癌组织中Nemo样激酶(NLK)表达与胆囊癌侵袭转移及患者预后的关系。
    方法: 收集2008年9月至2012年6月上海交通大学医学院附属新华医院26例及上海交通大学医学院附属同仁医院10例胆囊癌患者的手术切除标本。取肿瘤组织和与之配对的癌旁组织(取自手术标本切缘,经病理检查证实为正常组织)。应用免疫组织化学染色及Western blot法检测患者胆囊癌组织和相应癌旁组织中NLK的表达。采用门诊和电话随访,随访时间截至2013年5月。多样本比较采用χ2检验,双变量相关分析采用非参数Spearman等级相关分析,Kaplan-Meier法绘制生存曲线,生存分析采用Log-rank检验。
    结果:癌旁组织中NLK阳性表达率为25.0%(9/36),胆囊癌组织中NLK呈强阳性表达,NLK阳性表达率为61.1%(22/36),两者比较,差异有统计学意义(χ2=10.020P<0.05)36例胆囊癌患者的配对标本中,有25例患者的标本中NLK在胆囊癌组织中的表达高于癌旁正常组织,表达上调率为69.4%(25/36)。单因素分析结果表明:NLK的阳性表达与患者性别、年龄、组织分化程度、TNM分期及淋巴结转移无关(χ2=0.0430.1025.1862.3651.860P>0.05),而与肿瘤浸润深度有关(χ2=16.264,P<0.05)。36例获得随访患者中,NLK阴性表达的胆囊癌患者的中位生存时间为19.0个月,NLK阳性表达患者中位生存时间为8.0个月,两者比较,差异有统计学意义(Log-rank值=10.150,P<0.05)。结论:NLK在胆囊癌的发生、发展过程中发挥重要作用,NLK的高表达可能与胆囊癌的侵袭转移及患者预后密切相关。

     

    Abstract: Objective:To investigate the relationship between the expression of nemo-like kinase (NLK) and the invasion, metastasis and prognosis of gallbladder cancer.
    Methods:The gallbladder cancer tissues and adjacent tissues were obtained from 26 patients who were admitted to the Xinhua Hospital of Shanghai Jiaotong University and 10 patients who were admitted to the Central Hospital of Changning District from September 2008 to June 2012. The expression of NLK in the gallbladder cancer tissues and the adjacent tissues were detected by immunohistochemistry and Western blot. Patients were followed up via out-patient examination and phone call till May 2013. All data were analyzed using the chi-square test or non-parametric Spearman′s correlation coefficient for ranked data. The survival curve was drawn using the Kaplan-Meier method, and the survival was analyzed using the Log-rank test.
    Results:The positive expression rate of NLK in adjacent tissues was 25.0%(9/36). The expression of NLK in the gallbladder cancer tissues was strongly positive, with the positive expression rate of 61.1%(22/36). There was a significant difference in the NLK expression between the adjacent tissues and gallbladder cancer tissues (χ2=10.020, P<0.05). The expression of NLK in the gallbladder cancer tissues was higher than that in the adjacent tissues in 25 patients, with the expression up-regulation rate of 69.4% (25/36). The results of univariate analysis showed that the positive expression of NLK was not correlated with the gender, age, lymph node metastasis, tumor differentiation degree and TNM stage (χ2=0.043, 0.102, 5.186, 2.365, 1.860, P>0.05),but with the depth of infiltration2=16.264, P<0.05). The median survival time of patients with negative expression of NLK was 19.0 months, which was significantly longer than 8.0 months of patients with positive expression of NLK (Log-rank value=10.150, P<0.05).Conclusion: NLK plays an important role in the genesis and development of gallbladder cancer. High expression of NLK might be correlated with the metastasis and invasion of gallbladder cancer as well as the prognosis of patients.
