可吸收补片在腹腔镜腹壁疝修补术中的应用进展

Advances in the use of absorbable mesh in laparoscopic ventral hernia repair

  • 摘要: 腹腔镜技术因其创伤小、恢复快,已成为腹壁疝修补术的重要选择。传统不可吸收合成补片作为腹腔镜腹壁疝修补术最常用的补片类型,虽能提供可靠的力学支撑,但其长期存在于人体内可能发生补片侵蚀、皱缩导致肠瘘、腹腔粘连、术后异物感、慢性疼痛、感染等并发症,该现象日益受到关注。可吸收补片凭借其优异的生物相容性与诱导组织重塑能力,可降低上述并发症发生率,正在为腹腔镜腹壁疝修补术带来革新。笔者介绍现有可吸收补片的材料类型及其特点,重点讨论其在腹腔镜腹壁疝修补术中的应用进展、现存挑战及应对措施,分析其在腹腔镜腹壁疝修补术中的个体化手术决策。

     

    Abstract: Laparoscopic technology has become an important choice for ventral hernia repair due to its advantages of minimal trauma and rapid recovery. Traditional non‑absorbable synthetic meshes are the most commonly used type of mesh in laparoscopic ventral hernia repair. Although they provide reliable mechanical support, their long‑term presence in the human body may cause mesh erosion and wrinkling, leading to complications such as intestinal fistula, abdominal adhesions, postoperative foreign body sensation, chronic pain, and infection, which are receiving increasing attention. Absorbable meshes, with their excellent biocompatibility and ability to induce tissue remodelling, can reduce the incidence of the above complications and are revolutionising laparoscopic ventral hernia repair. The authors introduce the types and characteristics of existing absorbable mesh, focusing on their clinical efficacy in laparoscopic ventral hernia repair, existing challenges and countermeasures, as well as individualized surgical decision‑making in laparoscopic ventral hernia repair.
